Cancellation Policies

REFUND, CANCELLATION AND RESCHEDULING POLICY

The Aesthetic Medicine and Anti-Aging Clinics of Louisiana maintain a full practice and busy patient schedule, and the office does its best to accommodate the scheduling needs of all patients. However, untimely cancellations by patients and untimely rescheduling of appointments create additional financial and administrative costs and hardships for the office and for all patients. The adverse impact of untimely cancellations and/or untimely rescheduling is substantial, and the full extent of the impact is impracticable or difficult to determine and, therefore, to accommodate the scheduling needs of all patients, the patient agrees to the following:

SURGERIES FINANCED BY LOANS:

Patients who choose to finance procedures through outside financing will be required to put down a minimum of $1,000.00 deposit via credit card, cashier’s check or money order. This can be refunded after the procedure if the patient chooses to finance the entire procedure. In the event the patient cancels the procedure within 30 days prior to the scheduled date, the deposit is forfeited and is non-refundable.

CANCELLATIONS:

Refundable & Non-refundable Deposits.

If the patient cancels 30 days or more prior to the scheduled date, the patient shall be refunded the 20% deposit, minus a 5% charge in the case of payments made by credit card. Any request for cancellation must be submitted in writing. If the patient cancels less than 14 days prior to the scheduled procedure, the entire surgical fee shall be non-refundable. Patient is financially liable for these amounts.

Refundable & Non-refundable Procedure Fees.

If the balance of the procedure fee is not received at least 7 days prior to the scheduled procedure, the procedure will be cancelled, and the 20% deposit shall be non-refundable. Patient is financially liable for these amounts.

RESCHEDULING APPOINTMENTS:

Additional Charges for Rescheduling.

If the patient reschedules a procedure less than 14 days prior to the scheduled date and time, then the addition to any other charges which may apply, the patient shall pay an additional charge equal to 10% of the procedure fee for each rescheduled date and time. Patient is financially liable for these amounts.
